BridgePoint Connections LLC is an Enhanced Care Management (ECM) and Community Supports Community Hub dedicated to bridging the gap between people, healthcare, and the community resources they need to thrive. We provide person-centered care management to eligible Medi-Cal members at no cost, understanding that navigating healthcare and social-service systems can feel overwhelming. Our team meets each person with compassion, respect, and understanding of their unique circumstances, recognizing that people are more than a referral or a case number—they are individuals with strengths, goals, and a story.

We walk alongside every member through comprehensive needs assessments, individualized care planning, coordination of medical and behavioral health services, appointment coordination, and ongoing advocacy. Our Community Supports navigation connects members to essential resources including housing assistance, food and medically supportive nutrition, transportation, utility and financial assistance, personal care services, respite care, and connections to additional health and social-service programs.
